Job Title: Rehabilitation Therapist

Job Description:

We are seeking a skilled and dedicated Rehabilitation Therapist to join our team. The successful candidate will be responsible for conducting occupational therapy assessments for patients across various settings (inpatient, outpatient, or community). They will develop and implement patient-centered therapy plans based on assessed needs and goals.

  • Conduct occupational therapy assessments for patients across various settings.
  • Develop and implement patient-centered therapy plans based on assessed needs and goals.
  • Provide interventions to improve patients' independence in Activities of Daily Living (ADLs), upper limb function, cognitive rehab, or home environment adaptation.
  • Educate patients and caregivers on strategies to support recovery and self-care at home.
  • Recommend and train patients in the use of assistive devices or adaptive equipment.
  • Document therapy sessions, assessments, progress notes, and discharge plans in accordance with clinical guidelines.
