Job Description

We are seeking a skilled and compassionate Home Engagement Nurse to join our team. As a key member of our care delivery unit, you will play a vital role in providing personalized care and therapy to clients in their homes.

Your primary responsibility will be to conduct home assessments using our proprietary methods and tools. Based on the assessment, you will design tailored care plans for individual clients, ensuring that their unique needs are met.

You will also facilitate engagement with clients, adhering to their care plan, and perform nursing assessments to monitor their overall well-being. If necessary, you will communicate any wellness concerns to the client's primary caregiver or doctor.

To excel in this role, you must possess excellent observational, leadership, and problem-solving skills. You should also be able to multi-task, work independently, and be proactive in making our workplace better.

Required Skills and Qualifications
  • Enrolled or Registered Nurse with a minimum of 3 years of relevant nursing experience in general or private hospitals.
  • Diploma in Nursing or similar, plus licenses to practice nursing in Singapore.
  • Excellent communication and strong interpersonal skills.
  • Ability to write and communicate in English.
  • Computer literacy.
