We are seeking a dedicated wellness professional to deliver holistic care services that go beyond rehabilitation.

  • The incumbent plays a vital role in delivering holistic care to patients, focusing on their overall well-being.
  • Develop and promote wellness initiatives and activities at community dialysis centers.
  • Oversee patient screenings, conduct physical assessments, compile and analyze data, and provide recommendations for follow-up care.
  • Conduct regular reviews to track patient adherence and progress in exercise programs, ensuring continuity of care plans.
  • Ensure proper maintenance, disinfection, and servicing of exercise equipment and facilities.
  • Monitor and manage patient attendance, ensuring accurate and timely documentation to support consistent program participation.
  • Collaborate with clinical services teams to identify and address barriers to participation, ensuring patients receive adequate support in their wellness journey.
  • Support the planning and execution of volunteer events and social outings to empower and engage senior patients.

Requirements:

  • Prefarable Diploma in Health Sciences (Health Management) or other relevant courses (e.g., sports sciences, therapy aide, community care associate).
  • Relevant working experience in a healthcare setting is advantageous.
