Job Overview

We are seeking a skilled Healthcare Wellness Specialist to deliver holistic care to patients, extending beyond rehabilitation services.

Key Responsibilities:

  • Develop and implement wellness initiatives and activities at the dialysis center.
  • Oversee patient screenings, conduct physical assessments, compile and analyze data, and provide recommendations for follow-up care.
  • Conduct routine reviews to track patient adherence and progress in the exercise program, ensuring continuity of care plans.
  • Ensure proper maintenance, disinfection, and servicing of equipment used in the program.
  • Monitor and manage patient attendance, ensuring accurate and timely documentation to support consistent program participation.
  • Collaborate with the Clinical Services Team to identify and address barriers to participation, ensuring patients receive adequate support in their wellness journey.
  • Support the planning and execution of volunteer events and social outings to empower and engage senior patients.

Requirements:

  • Prefarable Diploma in Health Sciences or other relevant courses (e.g., sports sciences, therapy aide, community care associate).
  • Relevant working experience in a healthcare setting is advantageous.
