Job Opportunity

We are seeking a dedicated professional to play a vital role in our Integrated Community Care Provider (ICCP) team.

The selected candidate will be responsible for leading partnerships with ICCP providers, centre managers, and ground teams to implement sub-regional workplans, monitor progress, and report on results.

  • Develop and maintain effective relationships with stakeholders to enhance care integration and coordination collaborations.
  • Organise and facilitate regular meetings and check-ins for centre managers and ground staff to build understanding and address implementation challenges.

In addition to these responsibilities, the successful candidate will also:

  • Collect and consolidate information for quarterly progress reports;
  • Ensure timely submission of endorsed reports and claim forms;
  • Represent the sub-region at meetings as required and report back to leadership teams.
