Job Opportunity

We are seeking a compassionate professional to join our team as a Senior Social Worker. As a key member of our organization, you will play a vital role in providing care and support to individuals with dementia and their caregivers.

Key Responsibilities:
  • Conduct thorough assessments and develop personalized care plans for individuals with dementia and their caregivers.
  • Offer psychoeducation, supportive counseling, and referrals to relevant services as needed.
  • Maintain effective communication and collaboration with multidisciplinary professionals and external service providers to ensure comprehensive care management.
  • Document case recordings and maintain accurate records.
  • Participate in team meetings and case conferences to discuss client progress and plan future interventions.
  • Assist in data collection and statistical analysis for program evaluation and reporting purposes.
  • Facilitate caregiver support group sessions and participate in outreach events.
  • Perform other tasks as required to support the delivery of dementia care services.
Requirements:
  • A degree in an allied health profession, social work, or psychology is preferred; senior position may be considered for those with suitable experience.
  • Excellent communication, interpersonal, and written skills.
  • Ability to facilitate group sessions and work independently.
  • Possibility to work occasional Saturdays and weekday evenings.
  • Passion for dementia care and working with people.
  • Previous experience in dementia care is an added advantage.
