Job Summary:

We are seeking a highly skilled and compassionate Social Worker to provide comprehensive support to individuals with dementia and their caregivers.

  • This role involves conducting thorough case assessments, developing individualized care plans, and providing psychoeducation and supportive counseling.
  • The successful candidate will collaborate with multidisciplinary professionals, maintain accurate records, and participate in team meetings and case conferences.
  • Additional responsibilities include assisting in data collection and analysis for program evaluation and reporting purposes, planning and facilitating caregiver support group sessions, and supporting outreach events and talks.
Requirements:
  • Relevant degree in social work, psychology, or an allied health profession
  • Excellent communication, interpersonal, and written skills
  • Ability to facilitate group sessions and work independently
  • Flexibility to work occasional Saturdays and weekday evenings
  • Passion for dementia care and working with people
  • Prior experience in dementia care is an added advantage
