We are seeking a skilled and experienced physiotherapist to join our team as a home-based specialist. As a key member of our healthcare team, you will be responsible for providing high-quality physiotherapy services to patients in their homes.
">- The role requires conducting initial assessments, using relevant outcome measures to develop effective treatment plans, designing individualised care plans and goals tailored to each patient's needs, providing treatment interventions in a home setting, including caregiver training and equipment prescription, discharge planning, and referring patients to other services as needed.

Requirements:
">- Hold a degree or diploma in Physiotherapy recognised by the Allied Health Professions Council.
- Be fully registered with the Allied Health Professions Council.
- Demonstrate excellent interpersonal skills and independence in conducting assessments and interventions in a home setting.
- Ability to design individualised care plans tailored to each patient's needs.
