Job Description

We are seeking a skilled and compassionate Holistic Home Healthcare Professional to join our team. As a member of our care team, you will provide holistic, person-centred nursing to patients in their homes.

You will conduct home visits and palliative nursing assessments, develop and review individualised care plans to manage symptoms effectively, administer medications as directed by doctors, educate and support caregivers in providing end-of-life care, coordinate referrals to community services, and offer emotional and bereavement support to families.

  • Conduct regular home visits to assess patient needs and provide high-quality care
  • Develop and implement individualised care plans that meet patient needs
  • Administer medications and adjust care as needed based on doctor directions
  • Educate and support family members or caregivers in providing end-of-life care
  • Coordinate referrals to community services or inpatient hospices when necessary
  • Offer emotional support and bereavement counseling to patients and families
Requirements

To be successful in this role, you will need:

  • A diploma or degree in Nursing from an accredited institution
  • Palliative Care certification is an asset
  • A minimum of 3 years of experience in community or hospice nursing
  • Warm, empathetic, and professional approach with the ability to support families with sensitivity
  • Comfortable working independently and making decisions with minimal supervision
