Job Opportunity

We are seeking a skilled and compassionate healthcare professional to join our team. As a General Nurse, you will provide safe and high standards of care to patients in their own home setting, ensuring a seamless continuity of care after their discharge from the hospital.

About the Role
  • This is a challenging and rewarding position that requires strong communication and interpersonal skills.
  • You will work independently to execute nursing procedures in the patient's home setting, under the Hospital at Home programme with restructured hospitals, offsite location and medical centres.


Your Responsibilities
  1. To execute basic to complex nursing procedures in the patient's home setting, including medication administration via intravenous lines, withdrawing of blood samples, IV cannulation, physical assessment, vital signs monitoring, ECG monitoring, pain management, wound dressing, vaccinations, blood glucose monitoring, etc.
  2. To conduct patient assessment, provide appropriate nursing intervention and evaluate outcome of intervention, including escalating required situations to doctors.
  3. To effectively communicate patient's condition to internal and external healthcare team and provide accurate and timely updates via appropriate channels.
  4. To actively participate in handover meetings with partner hospitals to ensure seamless continuity of care.
  5. To provide caregiver training for patients and family members as required.
