Job Description

We are seeking a dedicated and passionate individual to provide case management and support to youths and their families. The ideal candidate will have a strong background in social work or counseling and be able to assess and implement intervention plans effectively.

Key Responsibilities:
  1. Conduct risk assessments, develop care plans, and provide appropriate levels of care to youths and their families.
  2. Collaborate with schools and community stakeholders to implement holistic intervention and supportive care plans.
  3. Facilitate psycho-education group sessions according to the provided curriculum.

Requirements:
  1. Minimum degree or postgraduate in Social Work or Counselling.
  2. At least 1-2 years' experience working with youth and families.
  3. Strong knowledge and ability to case assess, engage, and carry out intervention plans in a timely manner.
  4. Ability to work with clients in time-based programs to achieve desired outcomes.
  5. Excellent interpersonal skills and team player qualities.
