The role of a Senior Service Social Worker is pivotal in enhancing the well-being of seniors, promoting dignity and enabling ageing-in-place.

  • Provide person-centred case management, focusing on strengths and needs.

  • Perform psycho-social assessments to identify risks and opportunities for growth.
  • Collaborate with healthcare professionals, community partners, and caregivers to ensure seamless care coordination.

To succeed in this role, you will require:

Essential Qualifications
  • Bachelor's or Postgraduate Diploma/Master's degree in Social Work.
  • Strong passion for working with seniors and supporting caregivers in a community context.
  • Excellent interpersonal, communication, and organisational skills.
  • Ability to work independently while contributing to a collaborative team environment.
  • Proficiency in written and spoken English.
  • Fluency in local Mother Tongue language/dialects a plus.
