Job Title: Community Care Specialist

Job Description:

We are seeking a highly motivated and compassionate individual to join our team as a Community Care Specialist. In this role, you will conduct outreach and build partnerships with community organizations, healthcare providers, and social services to identify potential clients for our care management program.

You will organize and participate in outreach events, roadshows, and talks to promote our services and engage with the community. Your key responsibilities will include assessing clients' psychosocial, medical, and functional needs, developing and implementing personalized care plans, coordinating and linking clients to appropriate health and social care services, providing caregiver support and education, and monitoring and reviewing care plans to meet clients' changing needs.

As a Community Care Specialist, you will be an advocate for clients' best interests and promote quality of life. You will work closely with colleagues, external stakeholders, and community partners to provide holistic and coordinated care.

Required Skills and Qualifications:
  • Diploma in Social Work, Counselling, Psychology, Nursing, or relevant field.
  • Experience in case management, eldercare, or community-based services preferred.
  • Bilingual in English and a second language, dialect is an advantage.
  • Passionate about working with the elderly and their caregivers.
  • Comfortable conducting home visits and adaptable to different environments.
  • Meticulous and good time management.
  • Strong organizational skills and attention to detail in documentation and reporting.
  • Able to thrive in a fast-paced, dynamic, and multifaceted work setting.
  • Good interpersonal and communication skills to build rapport with clients and their families, as well as colleagues and external stakeholders.
