Compassionate Home-Based End-of-Life Care

Our team seeks a compassionate and skilled Nurse to provide dignified end-of-life care to patients in their homes. As a dedicated professional, you will bring comfort and support to those who need it most.
Key Responsibilities:
- Conduct home visits and palliative nursing assessments.
- Develop and review individualised care plans to manage symptoms effectively.
- Administer medications and adjust care based on doctor's directions.
- Educate and support caregivers in providing end-of-life care.
- Coordinate referrals to inpatient hospices or community services.
- Offer emotional and bereavement support to families.
- Maintain accurate documentation; target minimum of 6 visits/day.
- Support outreach and fundraising efforts when needed.
Required Skills and Qualifications:
- Registered Nurse with SNB (Diploma/Degree in Nursing).
- Palliative Care certification is a plus.
- Minimum 3 years of nursing experience (community/hospice preferred).
- Warm, empathetic, and professional — able to support families with sensitivity.
- Comfortable with independent home visits.
