Senior Community Outreach Coordinator Job Description

We are seeking a highly skilled and compassionate Senior Community Outreach Coordinator to join our organization. As a key member of our team, you will play a vital role in supporting the health and well-being of seniors in our community.

The primary responsibilities of this position include assisting with the planning and implementation of active ageing programmes, providing befriending and outreach services to vulnerable elderly individuals, and working closely with managers to achieve centre performance indicators.

Key Responsibilities:

  • Conduct community outreach and need assessments to identify seniors who require support
  • Provide befriending and buddying services to frail and vulnerable elderly individuals at their homes
  • Assist with the development and implementation of active ageing programmes
  • Collaborate with healthcare professionals and other stakeholders to ensure seamless service delivery
  • Perform administrative tasks such as data collection, reporting, and record-keeping

Requirements:

  • Minimum diploma holder
  • Experience in eldercare, healthcare, or social services
  • Able to speak dialects (e.g., Hokkien, etc.) and simple Malay desirable
