Job Title:

Community Care Coordinator

We are seeking a skilled and compassionate Community Care Coordinator to join our team.

As a key member of our outreach program, you will be responsible for conducting community-based programs to support the well-being of seniors.

Your primary goal will be to provide befriending and outreach services to seniors, ensuring their well-being and connecting them with necessary resources.

  • Assisting in the planning, coordination, and evaluation of community-based programs.
  • Providing one-on-one support to frail and vulnerable elderly at their homes.
  • Conducting community outreach and need assessments to identify areas of support for elderly individuals within designated service boundaries.
  • Referring elderly individuals to relevant help services when needed.
  • Performing administrative duties such as maintaining attendance records, visitation notes, and data collection as required by authorities.
Required Skills and Qualifications

To succeed in this role, you will possess a minimum Diploma qualification and experience in eldercare, healthcare, or social services.

Strong communication skills, both written and verbal, are essential. Additionally, proficiency in dialects (e.g., Hokkien) and basic Malay language is desirable.
