Community Care Integration Specialist

We are seeking a highly skilled Community Care Integration Specialist to join our team. This role will play a pivotal part in facilitating collaboration across partner organisations, aligning strategies, and supporting the implementation of sub-regional workplans.

This position is ideal for individuals with experience in programme coordination, care integration, or community partnership roles, particularly in the eldercare or social service sector. Strong interpersonal and communication skills, with the ability to engage and build relationships across multiple stakeholders, are essential.

Key Responsibilities:
  • To coordinate care efforts with various partners to ensure a seamless care journey for seniors in the community.
  • Work with leadership teams of partner organisations to align goals, establish roles, and develop strategies and workplans for the Sub-Region to achieve the deliverables.
  • Collaborate with centre managers and ground teams to implement the sub-regional workplan, while monitoring and reporting on progress.
  • Organise and facilitate regular meetings and check-ins with centre managers and ground staff to build understanding and address implementation challenges.
  • Engage and build networks with community care providers, regional health systems, grassroots organisations, and other stakeholders.
  • Partner with providers from the Sub-Region to pilot joint referral workflows in Phase 1 and support care coordination deliverables in Phase 2.
  • Work closely with funder's Sub-Regional Relationship Manager to track progress and report indicators.
Administrative Functions:
  • Collect and consolidate information required for quarterly progress reporting.
  • Ensure endorsed reports and claim forms are submitted to funder in a timely manner.
  • Represent the Sub-Region at meetings, and update the leadership teams of partner organisations on key discussions and outcomes.
Requirements:
  • Degree or diploma in Social Work, Nursing, Gerontology, Community Development, or related fields will be beneficial.
  • At least 3–5 years of experience in programme coordination, care integration, or community partnership roles (experience in the eldercare or social service sector preferred).
  • Strong interpersonal and communication skills, with the ability to engage and build relationships across multiple stakeholders.
  • Organised and detail-oriented, with proven skills in project management, monitoring, and reporting.
  • Able to work independently while fostering collaboration within a team setting.
