Job Title: Community Care Coordinator

About the Role

This is a challenging and rewarding role where you will be responsible for leading the implementation of community care initiatives in the Toa Payoh sub-region. You will work closely with partner organizations, centre managers, and ground staff to achieve the deliverables specified for the Integrated Community Care Provider.

You will be responsible for coordinating secretariat and coordination functions, including working with partner organizations' leaders, establishing roles, and developing strategies and workplans. You will also engage and build networks and partnerships with other community care providers, regional health systems, grassroots organizations, and other stakeholders.

In addition, you will assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners. You will also work with ICCP providers to trial joint referral workflows to support care coordination for seniors.

Key Responsibilities
  • Lead in working with partner organizations' leaders to align goals, establish roles, and develop strategies and workplans for the Toa Payoh sub-region.
  • Work with centre managers and ground teams to implement the sub-regional workplan and monitor progress.
  • Organize and facilitate regular meetings and check-ins for centre managers and ground staff to build understanding and work through implementation and other challenges.
  • Engage and build networks and partnerships with other community care providers, regional health systems, grassroots organizations, and other stakeholders.
  • Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners.
Requirements
  • Bachelor's degree from an accredited University, preferably in Social Work, Healthcare Management, Gerontology, or related field.
  • At least 5 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships.
  • Experience working with seniors or in senior care services.
  • Strong leadership, communication, and interpersonal skills, with the ability to build relationships and lead cross-functional teams.
  • Strategic thinking with an ability to align operational priorities with long-term strategic goals and ensure sustainable delivery of services.
