Job Description:

We are seeking a skilled and experienced professional to lead our community care team in the Toa Payoh sub-region.

The successful candidate will be responsible for working with partner organisations, centre managers, and ground staff to implement the sub-regional workplan, monitor progress, and report on outcomes. Key responsibilities include leading coordination efforts, building networks and partnerships, assessing resources, and identifying opportunities for care integration and coordination collaborations.

This role requires strong leadership, communication, and interpersonal skills, as well as strategic thinking and problem-solving abilities. The ideal candidate will have at least 5 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships and delivering results.

Key Responsibilities:

  • Lead in working with partner organisations' leaders to align goals, establish roles, and develop strategies and workplan
  • Work with centre managers and ground teams to implement the sub-regional workplan, and monitor and report on progress
  • Organise and facilitate regular meetings and check-ins for centre managers and ground staff
  • Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders
  • Assess resources and identify opportunities for care integration and coordination collaborations across partners and external partners
  • Work with ICCP providers to trial joint referral workflow to support care coordination for seniors
  • Represent the sub-region to attend meetings convened by AIC as required

Required Skills and Qualifications:

To be successful in this role, you will need:

  • A Bachelor's degree from an accredited University, preferably in Social Work, Healthcare Management, Gerontology, or related field
  • At least 5 years of experience in healthcare coordination, community care, or a related field, with a proven track record of managing partnerships
  • Experience working with seniors or in senior care services
  • Strong understanding of community care delivery, integrated service models, and inter-agency collaboration
  • Strong understanding of the senior care landscape and relevant industry regulations
  • Strong leadership, communication, and interpersonal skills, with ability to build relationships and lead cross-functional teams
  • Strategic thinking with ability to align operational priorities with long-term strategic goals and ensure sustainable delivery of services
  • A proactive, solution-oriented mindset focused on delivering results
