The role of a Senior Community Outreach Coordinator is to facilitate strong connections between seniors and community resources.

Key Responsibilities:
  • Stakeholder Management
    • Create detailed maps of community assets and services.
    • Promote outreach events, including door-to-door visits, needs assessments, and information-sharing sessions.
    • Become familiar with local community partners and engage in joint efforts to serve senior needs.
    • Communicate senior needs to external stakeholders and advocate for action.
    • Evaluate and recruit new seniors using established tools.
    • Tier new members according to set criteria.
    • Maintain and support volunteers for our programs and activities.
  • Day-to-Day Centre Operations
    • Distribute projects and activities to seniors at home or off-site, aligning with our Program Strategy.
    • Provide constructive feedback on operational gaps and suggest improvements for activity implementation.
    • Evaluate processes and offer suggestions for better implementation.
    • Cultivate suitable seniors to attend programs at our Activity Centre or their homes.
    • Organize, conduct, and assist in programs at the centre.
    • Document important senior information and data for reporting purposes.
  • Reporting
    • Submit timely check call reports and senior attendance records to meet funders' KPIs.
    • Conduct CST assessments and record them into our database.
    • Update senior data into our databases regularly.
    • Maintain periodic housekeeping of seniors' case status.

Requirements:

  • Minimum Diploma in Social Work, Gerontology, or Psychology
  • Able to work evenings and weekends when required
  • Proficient in MS Office applications (Word, Excel, PowerPoint)
  • Passionate about working with seniors
  • Strong organisational and communication skills
  • Meticulous and detailed
  • Good problem-solving skills
  • Enthusiastic and self-motivated
