We are seeking a highly skilled Occupational and Physiotherapy Professional to join our team. In this role, you will be responsible for developing personalized therapy plans for clients by conducting thorough assessments.
You will select outcome measures to track progress, collaborate with stakeholders, and create evidence-based interventions.
- Conduct occupational/physiotherapy assessments during client admissions and periodic care reviews
- Select and implement tailored outcome measures to gauge therapy progress
- Collaborate with clients and stakeholders to develop achievable goals
- Design evidence-based therapy interventions prioritizing individual client needs
- Maintain accurate documentation of assessment results and therapy plans
- Effectively communicate assessment findings and intervention plans to clients and relevant parties
- Provide recommendations for assistive devices and equipment as needed
- Develop comprehensive discharge and continuity of care plans
- Foster community integration through strategic partnerships
- Contribute to case discussions within the care team
- Offer guidance and support in training Therapy Assistants
Requirements:
- Degree in Physiotherapy or Occupational Therapy
- Registration with Singapore Allied Health Professional Council (AHPC)
