Job Overview:

We are seeking a highly skilled Care and Support Specialist to join our team. This is an excellent opportunity for someone who is passionate about providing exceptional care and support to individuals in need.

About the Role:

This role will involve working closely with stakeholders to develop and implement effective care and support plans. You will be responsible for ensuring that all aspects of care and support are coordinated and delivered in a timely manner.

You will also be responsible for managing critical incidents, including developing and implementing safety plans and coordinating follow-up care and support.

In addition, you will capture and maintain care and support information, data, and records for analysis and reporting.

Requirements:

To be successful in this role, you will need to have:

  • A bachelor's degree in a related field, such as psychology or social work
  • At least 6-8 years of experience working directly in care and support services
  • Proven track record in case management and crisis management
  • Excellent communication and interpersonal skills
