Healthy Community Developer

Healthy Precincts are geographical locations with environments and cultures that support residents to achieve and sustain healthy lifestyles. To contribute to this vision, we seek a highly motivated individual who can work collaboratively with internal and community stakeholders to develop and operationalize plans for empowering communities to take care of their health.
About the RoleThe successful candidate will be involved in the development and implementation of community health plans for the precincts under their care, with emphasis on preventive health and social-health integration. This includes:
- Supporting the planning and operationalizing of Health Precincts concept to achieve health-social integration with internal and community stakeholders.
- Developing and implementing community health plans to empower communities to take care of their health.
- Engaging and building partnerships with internal and community stakeholders, including grassroots organizations, social service agencies, and healthcare providers, to keep patients and residents healthy and integrated across care settings.
The ideal candidate should possess strong planning, project management, and organizational skills, as well as the ability to think critically and communicate effectively. A bachelor's degree in any discipline and at least 5 years of working experience are required.
