OverviewSupport the operations of the Community Care Team in the assigned area to improve health and anchor care in the community.
Job Responsibilities- Support the operations of the Community Care Team in the assigned area to support residents in leading a healthy lifestyle, including community operations such as transition care programmes and Community Health Posts.
- Engage community partners to collaborate on resident care and coordinate community operations.
- Assist in operations planning and data analysis, monitor workload and KPIs.
- Support improvements to community operations to enhance care and resident experience.
- Pilot new programmes and processes, evaluate technology use, and assess pilot success for future implementation.
- Contribute to Patient Experience initiatives, managing feedback and working with stakeholders for service recovery and gap analysis.
- Perform other duties as assigned.
- Bachelor's Degree
- Minimum 2 years of working experience
- Healthcare, operations, or customer service experience is advantageous
- Good communication, interpersonal, and written skills
- Strong responsibility sense and complaint management skills
- Seniority level: Mid-Senior level
