As a Case Manager RN, you will play a crucial role in driving Referral Management and coordinating appropriate sites of service. Your responsibilities will include facilitating Care Transitions and providing Care for High-Risk patients. Collaboration with PCPs, Specialists, and Spectrum Clinical Pharmacists will be essential to ensure coordinated service delivery. This position integrates pharmacy management, care coordination, utilization management, and care transition management.You will have the flexibility to telecommute from anywhere within OH, IN, KY, IL, and MO. The work hours are Monday to Friday from 9 AM to 5 PM, with the ability to work 2 days until 7 PM.Your primary responsibilities will involve working with a Care Team to support patient and provider engagement for Care Transitions Management, Care Coordination, and Medication Therapy Management. You will facilitate the sharing of inpatient discharge information to the Care Team and Primary Care Offices using appropriate technology tools. Additionally, you will consult with patients post-inpatient discharge and schedule follow-up appointments, educate patients on alternative sites of care, and engage high-risk patients telephonically for Comprehensive Medication Reviews. Furthermore, you will support high-risk patients by scheduling appointments with PCPs and specialists, provide benefit information and resources upon request, and communicate with provider offices to problem-solve patient issues.You will also be responsible for notifying PCPs and Clinical Pharmacists of relevant clinical information gathered during patient calls, developing strong relationships with provider offices, and facilitating meetings for initial onboarding and ongoing collaboration.Qualifications:- Bachelor's Degree or higher- Unrestricted, compact licensure as a Registered Nurse for applicable states or the ability to obtain RN license in OH, IN, KY, IL, MO- 4+ years of Case Management and/or Care Coordination experience- 3+ years of experience in managing populations with complex medical or behavioral needs- Intermediate knowledge of Microsoft Office, Outlook, and Teams applications- Ability to travel up to 10% to local offices- Flexible schedule, including evenings up to 7 PM- Preferred qualifications include a Clinical or Behavioral Health degree, experience in managed care or population health management, healthcare setting background, and Spanish/English bilingual proficiencySoft Skills:- Strong communication skills- Ability to provide proactive clinical feedback- Initiative-taking and independent work capability- Adaptability, flexibility, and incorporation of process changes in a dynamic environmentIn return, you will receive competitive compensation and benefits, including medical plan options, dental and vision insurance, 401(k) savings plan, education reimbursement, employee discounts, and more.
