Care Coordination and Management Professional
This role plays a pivotal part in driving referral management and coordinating suitable care settings. A skilled case manager will have a strong background in care coordination, utilization management, and case management.
Responsibilities involve facilitating transitions for patients with complex needs, collaborating with primary care physicians, specialists, and clinical pharmacists to ensure seamless service delivery.
The position requires working closely with a care team to support patient and provider engagement for care transitions management, care coordination, and medication therapy management.
Key responsibilities include:
- Utilizing technology tools to share inpatient discharge information with the care team and primary care offices.
- Consulting with patients post-inpatient discharge and scheduling follow-up appointments as needed.
- Educating patients on alternative sites of care and engaging high-risk patients telephonically for comprehensive medication reviews.
- Scheduling appointments with primary care physicians and specialists, providing relevant information and resources upon request, and communicating with provider offices to address patient issues.
- Notifying primary care physicians and clinical pharmacists of pertinent clinical information gathered during patient calls.
Requirements:
- Bachelor's degree or higher.
- Unrestricted compact licensure as a registered nurse for applicable states or the ability to obtain RN license in OH, IN, KY, IL, MO.
- 4+ years of experience in case management and/or care coordination.
- 3+ years of experience managing populations with complex medical or behavioral needs.
