You will be a part of HPB’s Regional Health & Community Outreach division which empowers residents in Singapore to attain optimal health, increase their quality and years of healthy life, and prevent illnesses, disability and premature death. To achieve this, HPB drives national health promotion and disease prevention programmes, and creates a supportive environment in Singapore where healthy lifestyle options are available and accessible to residents every day.

**Scope**

To support the Board’s goals, the Regional Health Systems and Community Outreach Division collaborates closely with the community and healthcare partners to co-create regional and community-owned solutions to ensure good coverage and sustainable delivery of priority health promotion programmes in the community setting.

Key programme areas include active ageing, physical activity, nutrition, substance abuse and mental health. The initiatives cover the general population segments from children to seniors, as well as targeted outreach initiatives to ethnic minority groups and disadvantaged groups (such as those from lower socioeconomic status).

Scope of work will include but not limited to:
**Building Healthy Community Ecosystems**
- Plan, implement and champion sustainable Healthy Community Ecosystems (HCE) for the population under your charge so as to amplify our health promotion efforts.
- Collaborate with key partners from government and non-governmental organisations, Regional Health Systems, community partners, volunteers and vendors, to increase outreach to the population segments and strengthen quality of programmes/services offered in the community setting. Develop a systematic approach to partner outreach and relationship management, and be able to maintain strong relationships with them so as to grow the partnership level.
- Work closely with internal programme teams to create and deliver programme packages suitable for your region/s.
- Manage and supervise day-to-day operations of health promotion programmes delivered by outsourced vendors to ensure smooth implementation in the community.
- Responsible for your region’s targets, and prudently manage budget under your charge.
- Forecast, measure, and report the results of various projects to the Division, Senior Management, internal and external stakeholders.

**Build up target outreach efforts to the ethnic minority communities**
- Develop and implement strategies to effectively reach out to the ethnic minority communities to promote a healthy lifestyle. These plans may include outreach initiatives and community event planning.
- Build relationships with ethnic community leaders, organisations, and stakeholders to collaborate on outreach efforts and partnerships; and raise awareness about HPB’s services and resources within the communities.
- Collaborate with WOG agencies and within HPB departments and divisions to ensure that programs and services tailored to the ethnic minority communities are accessible, inclusive, and meet their needs.
- Collect data and feedback from the communities and partners to assess the effectiveness of outreach efforts and refine future strategies.
- Previous working experience in a role similar to outreach and partnerships management for at least 3 years;
- A very good understanding of partnership development and relationship management. Ability to network easily and build strong collaborations with stakeholders;
- Excellent communication and interpersonal skills and versatility in dealing with different types of partnerships;
- Strong organisational, project management and time management skills; good multitasking skills but with attention to details;
- Ability to work independently, and with teams;
- Passion to serve, particularly in the community setting
