Social Worker, Case Manager is responsible for the delivery of care to clients with mental health issues, including assessment, casework management, counselling, psychoeducation and caregiver support and to ensure a holistic care approach, he or she may also be required to collaborate with other community agencies, based on the needs of the clientsConduct assessment and provide intervention and support to clients with mental health issues eg. psychoeducation, case management and counselling.Provide comprehensive mental health assessment, case management and psychosocial interventionsResponsible for maintaining client Individual Care Plan that are accurate and timely for submissionAssist with Home projects or report submissions, as assigned by supervisorNetwork with stakeholdersCarry out any other duties as assigned by HOD or supervisorSupport clients through full spectrum of case management work; including co-ordinating various support services, regular follow-up and communication.Build good rapport with relevant stakeholders and external agencies in order to mobilise resources to meet client’s needs.Complete assessments and reports and any other administrative requirements
