A case worker in a youth home plays a vital role in supporting our residents by helping them navigate their emotional, social, and educational challenges.

**Responsibilities**:

- Conduct assessments to determine the needs, strengths, and challenges of youth entering the home
- Develop personalized care plans based on the needs of each youth, regular monitoring and evaluating the progress, adjust care plans as necessary to ensure the best outcomes.
- Offer crisis intervention and support to help manage difficult situations or conflicts.
- Work closely with vaious stakeholders
- Attend meetings with other staff to discuss the well-being and progress of youth residents.
- Maintain accurate and up-to-date records of youth's progress, incidents, and interactions.
- Prepare reports for reviews, meetings, and other legal or administrative purposes.
- Connecting youth to aftercare services when necessary as they transit out of the youth home.
- Any other ad-hoc responsibilities assigned

- Problem solving abilities
- Have passionate to work with Youth Boys
- Rotating and Weekend Shifts
