Case Worker

On behalf of our client who is in the social service space, we are looking for qualified individuals to join them as social workers:
The Social Worker is responsible for facilitating change and improving lives for individuals, families, communities facing transitional challenges through the provision of direct services.
**Responsibilities**:
1. Conduct casework management, including gathering information, conducting the psycho-social assessment, identifying common needs and goals for intervention
2. Provide basic counseling as required
3. Plan, organize and implement community outreach activities, programs, and group work
4. Perform evaluation and prepare reports for administrative investigation and intervention programs
5. Provide information and referral services to link Clients to appropriate social services
6. Participate in research
**Qualifications**:
- Possess Degree in Science of Sociology / Gerontology / Psychology / Counselling or any related discipline from an accredited institution
**Relevant Experience**:
- Prior experience as a Social Work Associate / Case Worker would be an advantage
- Fresh graduates may welcome to apply
**Characteristics and Traits**:
- People-Oriented and have good interpersonal skills
- Excellent oral and written communications skills
- Analytical ability & problem-solving skills to analyse information quickly and accurately, and formulate decisions and make recommendations
- Committed and can-do attitude
- Effective report writing skills
- Team Player
