**1. **Reporting Officer**
You will report to the Team Lead for all matters relevant to this role.
**2. **Duties**
**i. **Case Management**
a) Conduct regular assessment of mothers and children enrolled in KidSTART using assessment tools
b) Develop **child-centric case plan **for each child identified with moderate/high needs according to the KidSTART protocol for follow ups
c) Monitor and track attendance at preschools and address any barriers to attendance (which may require engaging parents via home visits, at pick-up/ drop-off timings at preschools, or other parent engagement platforms organised by preschool)
d) Attend and convene case conferences, where necessary, with preschool teachers and principals, KidSTART MDT, and other agencies (e.g. Social Service Offices, Family Service Centres, etc.) to coordinate support for children and their families at a systemic and case level (e.g. ComLink)
e) Provide last mile delivery of support to families (e.g. accompanying parents and their children for further health checks)
f) Support and ease transitions across KidSTART components, transitions to primary school, and withdrawals from preschool and KidSTART
g) Document case plans and notes of meetings/case conferences
h) Work with other KidSTART practitioners for families with children / the same child enrolled in multiple KidSTART components to ensure that there is a clear key worker and coordinated case plan
i) Support the needs of other children between 0 - 6 years old in the family if they are not known to KidSTART
**ii. **Outreach**
a) Organise, execute and participate in outreach activities to find and recruit potential KidSTART families or families who need assistance with preschool enrolment together with volunteers.
b) Record the outcomes of outreach to determine status of targeted children.
**iii. **Intake and Referral**
a) Receive and process referrals from a variety of sources.
b) Seek consent from and determine the eligibility of families for KidSTART programme components.
c) For families in the region who are not eligible for any KidSTART programmes, the KSA will refer them to relevant help agencies.
d) Collate and maintain accurate and complete set of data in SSNet on children enrolled to KidSTART programme components and/or preschool.
**iv. **Engagement**
a) Support _KidSTART Programme Office _in mapping of community needs and assets.
b) Collate needs from KidSTART families overseen by the KSA for possible sponsorship or donation opportunities.
c) Assist with disbursement and tracking of sponsored items to KidSTART families.
**3. **Job Requirements**
a) Bachelor's degree in Social Sciences or related field.
b) Keen to work with families and young children and willing to go the extra mile to support families in need
c) Good writing skills for case management and comfortable with data collation for reporting purposes.
d) Strong organisational skills, with an ability to manage multiple projects and meet deadlines effectively.
e) Excellent communication and interpersonal skills, with an ability to work collaboratively with various stakeholders including parents, caregivers, teachers, and community partners.
f) A passion for uplifting individuals and families in vulnerable or disadvantaged situations, which involves a keen interest in understanding and forming relationships with individuals in need of support while adhering to professional boundaries.
g) Proficiency in Microsoft Office
h) Able to work on irregular hours due to home visit scheduling.
