**What you'll be doing:

- Conduct psycho-social and financial assessments, recommend and carry out appropriate clinical interventions, bereavement care and case management for residents and families.
- Initiate and manage evidence-based interventions.
- Networking with government, community and social agencies.

**JOB REQUIREMENT**
- Degree in Social Work from a recognised University approved by the Singapore Association of Social Workers
- Accredited Social Worker with the Singapore Association of Social Workers

**Competencies:

- At least 5 years' experience working in a healthcare environment providing assessment and case management. Experience in the ILTC sector and Palliative Care is much preferred.
- Strong team player who enjoys both clinical and community work
- Superior interpersonal, organisational and communication skills
- Competent administrative skills and adequate IT skills in maintaining updated documentation of case records and timely reviews of financial assistance
