**DESCRIPTION**

NOVI Health is a medical and digital health provider which aims to help people prevent, manage and reverse chronic lifestyle diseases such as diabetes and high cholesterol, and to live healthier lives. Our holistic approach combines lifestyle interventions with medical management, supported by data and technology, to deliver solutions at scale and improve health outcomes.

**JOB DESCRIPTION**

The Health Coach works closely with the principal physician and a multi-disciplinary care team to help clients improve their health and wellness, with a focus on fitness and nutritional assessment, education and appropriate evidence-based, personalised interventions. This involves both in-person and digital modalities, to support clients in achieving their health and fitness goals. The Health Coach will also develop and deliver client educational materials, workshops and programmes.

**ROLE**
- Provide one-on-one fitness and nutritional assessment, health coaching, education, goal-setting and support for clients, through various modes of communication (e.g. face to face, video/phone calls, messaging), taking into consideration clients’ readiness to change as well as biological and physiological profiles, to help them achieve their health and fitness goals
- Instruct clients in the use of devices and technologies e.g. glucose sensors, apps
- Collaborate with care team to promote and integrate high quality client-centred care
- Respond to clients’ queries, proactively monitor their progress and reach out to them as needed and appropriate
- Document client interactions in the electronic medical records system
- Develop protocols and training materials for health coaching

**REQUIREMENTS**
- Relevant Professional Certificate, e.g. American Council of Exercise, National Academy of Sports Medicine, International Sports Sciences Association, Precision Nutrition Certification in Nutrition and Coaching
- Relevant diploma or degree e.g. sports science, exercise science
- Experienced in providing health and fitness coaching to clients with different goals e.g. lifestyle / weight management, improving physical condition
- Keen interest in the use of digital technology to motivate and empower clients
- Able to communicate clearly and concisely, both in oral and written form, and able to tailor messaging to different populations in a culturally sensitive manner
- Excellent customer service
- Able to work independently and as part of a multidisciplinary team
