**Responsibilities**:

- Support in review and enhancements of care planning, activity and harnessing environment to empower PWD to live in wellness.
- Contribute as a member of the Evidence-Based Practices (EBP) team to measure impacts and co-create knowledge in dementia care.
- Co-lead, develop, implement, and benchmark activities such as mindfulness initiatives for PWDs, family members and staff.
- Teaming with Social Worker to provide end-to-end case management support and appropriate level of care through person-centred admission to Long Term Residential Care and Community Care, assessment, care planning and support for caregivers.
- Empower family members living with people with dementia and mild cognitive impairment in the community through caregiver mastery initiative.
- A good Bachelor or Master Degree in Psychology, Counselling or related discipline from a recognised university.
- Preferable with minimum 3 years of relevant experience.
- Excellence in conceptualization, writing and planning strategically leading to quality implementation, sense-making of data and documentation.
- A visionary, mature, caring, compassionate and pro-active team player
- Possess strong interpersonal skills.

**Job Types**: Full-time, Permanent

**Salary**: $3,000.00 - $3,800.00 per month
