**JOB SUMMARY**
The Senior/Executive will support the Population Health Office’s Assistant Manager/Manager in population health planning for Central Health, and support the development and implementation of population health management strategies (including the Central Health Model of Care (4 Principles, 6 Strategies)) at the Hospital and Central Health level.

**Main Duties & Responsibilities**:

- To support the Assistant Manager/Manager in keeping abreast of the local, national and international population health landscape and best practices, and conduct comprehensive literature reviews of relevant topics. Where suitable, to recommend suitable population health management tools and strategies for Central Health to the department and division leadership.
- To support the Assistant Manager/Manager in ensuring alignment of Central Health’s needs with the adopted population health strategies and service planning, and ensuring a strategy that is aligned to frameworks stipulated by Ministries or the cluster (National Healthcare Group).
- To support the Assistant Manager/Manager in the development of the Central Health Integrated Care Organisation (ICO) construct, including but not limited to service planning, evaluation of care and risk segmentation models, and planning of population health registry.
- To support the Assistant Manager/Manager in facilitating strategic dialogues between Hospital, Division and Department leadership with relevant Ministry, statutory board, community partners and cluster stakeholders on the Population Health Management approach for central zone.
- To work with the Assistant Manager/Manager in monitoring identified population health outcomes to inform service planning, and perform periodic reviews of key indicators for relevance.
- To support the Assistant Manager/Manager to engage internal and external stakeholders to drive, rationalize and align efforts to the Central Health Model of Care and any identified population health strategy.
- To work with the Assistant Manager/Manager to communicate and obtain buy in from internal and external stakeholders on Central Health’s population health strategies. This includes (but not limited) to publications, poster presentations, internal and external sharing platforms (e.g. MOH-NHG, NHG SMM, TTSH Management Councils, etc.).
- To support the Assistant Manager/Manager in supporting the division’s Enterprise Risk Management (ERM) through coordination, reviewing and recommending risk management (e.g. development of strategy and plans) for identified risks.
- To provide secretariat support where necessary.
**JOB REQUIREMENTS**:

- A bachelor's degree in medical, nursing or allied health sciences. Good degrees in other disciplines can be considered. Having a Masters Degree in Public Health would be advantageous
- 1-4 years of relevant work experience in population health, healthcare planning, strategic analysis, or public health
- Excellent working knowledge of Microsoft Office and experience in data analytics and/or primary and secondary research
- Strong written, analytical and communication skills
- Good people management skills and ability to manage multiple projects with effective time management
- Ability to work well independently as well as a team player
