Executive (Diabetic Foot Coordinator)

Under the ambit of the National Healthcare Group (NHG) Diabetes Steering Committee’s (DmSC’s) Dm Foot workgroup, DEFINITE (DiabEtic Foot IN prImary and TErtiary) Care, is one of the integrative-care initiatives in which Dm-complications of foot ulcers are pre-empted and addressed via a coordinated multi-disciplinary team implementation. Programme-wise, the role of the Diabetic Foot Coordinator (DFC) works closely with the NHG Polyclinic Family Physicians, Vascular Surgeons, Podiatrists and fellow DFCs at Tan Tock Seng Hospital, Khoo Teck Puat Hospital and NHG Polyclinics to assist in coordinating, delivering care and risk-stratifying Dm-patients with diabetic foot conditions.

**Major Duties & Responsibilities**

Clinical
- Demonstrate proficient knowledge of
- Diabetic related self-care management
- Diabetic foot assessment & self-care
- Clinical practice guidelines on Diabetic management, including DM foot
- Key measures of DEFINITE programme
- Care coordination, community resources and utilization review with respect to diabetic foot cases
- Demonstrate ability to
- Access institution systems to identify cases for management accurately & timely
- Assess patients for medical, psychological, social, environmental, vocational and financial needs
- Formulate plans of care in collaboration with clinicians and other health care members
- Perform care coordination practices; including assessment, monitoring, intervention & review of patients’ needs & care plans and education/coaching throughout the continuum of care
- Collaborate with various members of the multi-disciplinary team to coordinate care for the patients
- Coordinate & consolidate outpatient and primary care appointments with team and patients (all appointments, not only DM foot related) to improve the adherence rate
- Coordinate and collaborate with appropriate services to resolve practice issues and intervene as necessary when variances are identified
- Document care coordination activities accurately & timely
- Conduct the quality of life questionnaires
- Identify system, practice and patient issues & implement appropriate remedial action to address deficiencies found.
- Capture data accurately & timely according to the requirements of the DEFINITE Programme
- Generate reports accurately
- Function as a clinical resource person to all members of health care team
- Ensure safe patient care and cost-effective outcome
- Maintain good working relationship & contact with patient/family members and all service providers involved in care of patients
- Uphold strictly to data protection act, and protect information confidentiality at all times.

Quality Improvement and Academic Research
- Participate with team members to systematically address opportunities to improve clinical service and care
- Participate in the development, implementation and evaluation of new DM-complication abatement strategies, procedures or systems at hospital/national level.
- Participate in the DM foot related research projects and adhere to the protocols
- Assist in administrative duties relating to the setup of the clinical studies such as the preparation of documents and forms for ethics submission, ensuring that study paperwork is handled punctually and appropriately.
- Ensure that ethics reviews/observations are tracked and necessary actions are taken in a timely manner.
- Provide secretariat support for related meetings, etc.
- Uphold strictly to data protection act, and protect information confidentiality at all times.

**Job Requirements**

Education/Experience
- Degree/ Diploma in sciences or health related field
- Minimum 2 years of operations / clinical / research related experience in healthcare sector
- Prior experience with the clinical management of diabetic foot conditions is an advantage
