**About PeachCare Move**

PeachCare Move is a digital joint pain management program that is guided by physiotherapists and supported by health coaches. A multidisciplinary approach to care that improves treatment program experience and outcomes and supports associated lifestyle behaviour change.

**The role**

The health coach’s role is to support clients participating in our joint pain management program by providing guidance and encouragement to help reinforce their treatment program and facilitate client-centered lifestyle and behavior change to help clients achieve their treatment goals.
- Recognised health coaching certification
- OR have a nationally recognized diploma in health and wellness coaching
- Be optimistic and hardworking with a love for health and well-being
- Have a passion and enthusiasm for helping people become the most empowered version of themselves and reach their health goals
- Be a natural listener and motivator and can work with different individual needs through times of change
- Have excellent communication skills with the ability to utilize diverse modes of communication
- Can drive client engagement and outcomes leveraging new technologies
- Be confident with tech, can navigate apps and online systems, and comfortable learning new technology systems quickly and with ease
- Have strong attention to detail and strong organizational skills to manage your workload independently, document and note keeping, and be willing to ask for help from colleagues for support when needed
- Be able to develop positive, collaborative and professional relationships
- Be highly motivated, with a growth mindset and positive attitude where you are willing to learn and improve
- Have the ability to work remotely and privately.

**Desirable requirements**
- Experience working with clients directly in the following areas: health coaching, motivational interviewing, using and/or implementing various behaviour change techniques
- Ability to communicate in any additional languages to English specifically Mandarin, Malay, Tamil and/or dialect

**Responsibilities**:

- Manage the client throughout their treatment program from onboarding, one-to-one coaching, and tracking their progress and outcomes
- Provide remote health support using a customized digital platform as your main source of communication (video consultation and text-messaging chats)
- “Accountability buddy” - partner and collaborate with clients to co-create personalized healthcare plans to support their prescribed treatment program and achieve their short and long-term goals
- Provide coaching and education to assist clients in implementing their personalized healthcare plan
- Support clients as they learn to recognize their own values, strengths and insights and mobilize those assets to help them reach their goals
- Help clients recognize barriers and facilitate self-discovery in overcoming them
- Evaluate and track health data to understand clients’ challenges that could affect their treatment and health outcomes
- Collaborate and communicate with relevant Healthcare Professional staff in a timely, and effective manner to ensure client progress is appropriately monitored, referring to other healthcare professionals as needed
- Track client progress and monitor client data, maintain accurate records and manage individual caseloads efficiently

As part of your duties at Peach Heath Asia, you may come in contact with sensitive client information. You will be expected to follow our confidentiality, safeguarding, security and privacy procedures to ensure our patients' information is handled appropriately. Training will be provided.
