Palliative Care Nurse

As a Palliative Care Nurse (Home Care), you will be responsible for the following duties:

  • Provide compassionate nursing care to patients with life-limiting illnesses in their homes.

  • Assess and manage symptoms such as pain, breathlessness, nausea, and anxiety.

  • Administer medications and perform nursing procedures in line with care plans.

  • Offer emotional support and education to patients and caregivers.

  • Collaborate with doctors, allied health professionals, and social workers to ensure holistic care.

  • Monitor patient condition and update care plans as needed.

  • Support end-of-life care, ensuring comfort and dignity for patients.

  • Maintain accurate nursing records and documentation.

  • Educate caregivers on basic nursing tasks and symptom management.

  • Participate in case discussions, reviews, and multidisciplinary meetings.

Requirements:

  • Possess a Diploma/ Degree in Nursing with a valid registration with Singapore Nursing Board (SNB)

  • Advanced diploma in Palliative Care will be an advantage

  • Minimum 5 years of nursing experience
