Job description:

MAJOR DUTIES & RESPONSIBILITIES

Specific

Case Management / Psychosocial Care

  1. Staff will provide psychosocial care to patient and their significant others in Singapore's multi-cultural context as directed by the Medical Social Worker
  2. Will review intervention plans and work with the management of the case together with the Medical Social Worker.
  3. They will conduct independent assessment of straight forward and non-straight forward financial cases and apply for relevant national financial schemes for patient and their family members
  4. Prepare brief social assessment for the Medical Social Worker for continuation of follow up
  5. Conduct home visits and organizations engagement independently and provide assessment of the home situations or organizations to the Medical Social Worker
  6. Staff will need to meet department clinical indicators.
  7. Member of Quality improvement related projects that will improve patient's outcome.

Group Work

  1. Assist in logistics support and co-facilitate group work sessions for patients/ significant others as directed by the Medical Social Worker

Community Work

  1. Contribute in community mapping and ground engagement as directed by the Medical Social Worker
  2. Plan and work with the social work coordinator to plan for community events

Education / Clinical Supervision

  1. Participate as a member of Department's education or orientation team.

General

  1. To apply and adhere to social work's professional ethics and values
  2. To be actively involved in Department's initiatives
  3. Any other duties as assigned by the team leader or Supervisor and Head of Department

JOB REQUIREMENTS

Education / Training / Experience

  1. Minimum GCE 'O'/'A' Levels/ NITEC in Community and Social Services
  2. Candidates with 2 years of relevant social service will be an advantage.
