Job Responsibilities:

  • Conduct pre-admission screenings and support nurses in monitoring patient conditions.
  • Communicate with patients/caregivers, providing updates, education, and lifestyle advice.
  • Identify and escalate care gaps or abnormal findings to the clinical team.
  • Maintain accurate patient records and assist in care planning with interdisciplinary teams.
  • Coordinate referrals and continuity of care with community partners and healthcare providers.
  • Contribute to quality improvement, risk management, and departmental projects.

Requirement:

  • Diploma in a health-related discipline preferred.
  • Prior healthcare or hospital experience is an advantage.
  • A good team player, meticulous, service-oriented
  • Able to work in fast paced environments
