As a Principal Care Coordinator (Healthcare), you will be responsible for the following duties:
- Leader of the team, responsible for patient and family support services.
- Ensure optimal health outcomes and enhancing home-based coping mechanism.
- Conduct follow-up via phone or home visits to ensure patients and caregivers are managing effectively.
- Coordinate community services and provide education on caregiver responsibilities, including end-of-life care planning.
- Supervising data systems, such as managing the input, maintenance and analysis of data and data systems to support patient care and program evaluation.
- Collaborating with Acute and Integrated Long Term Care settings by working closely with partners to strengthen post-discharge support. aiming to reduce unnecessary hospital readmissions and ensure seamless care opportunity.
- Creating and implementing programs to meet the needs of patients and caregivers within the community.
- Manage staff performance, ensuring clinical competence and adherence to best practices.
- Plan and oversee the professional development and training requirements of the team to maintain high standards of care.
- Evaluate and refine operational processes in collaboration with internal stakeholders and external organisations to enhance quality care and operational efficiency.
- Participate and lead hospital initiatives, such as quality improvement projects, LEAN projects or research efforts, to contribute to the enhancement of clinical care quality.
Requirement:
- Possesses a professional qualification in Nursing / Social Work / Allied Health or relevant clinical disciplines.
- At least 8 years of experiences as a Care Coordinator, Case Manager in either acute or community healthcare environments.
- Demonstrates a minimum of 3 years of experiences in team leadership roles.
