We are seeking a compassionate and proactive Homecare Executive to join our team. This role is pivotal in coordinating and delivering high-quality homecare services to clients, ensuring their well-being and satisfaction. You will work closely with clients and healthcare professionals to provide personalized care solutions that meet individual needs.

Client Engagement & Care delivery

  • Conduct initial assessments of clients' health, living conditions, and homecare needs.
  • Develop and implement individualized care plans in collaboration with clients, families, and healthcare professionals.
  • Coordinate and monitor the delivery of homecare services, ensuring alignment with care plans and client expectations.
  • Provide assisted living care such as showering, toileting, transfer from wheelchair to bed, feeding etc.
  • Serve as the main point of contact for clients and caregivers, addressing inquiries, feedback, and concerns promptly.
  • Promote awareness of homecare services through outreach and engagement activities.

Operational & Administrative Duties

  • Maintain accurate and up-to-date documentation of client records, care plans, service logs, and incident reports.
  • Ensure compliance with internal policies, industry standards, and regulatory requirements.

Quality Assurance & Improvement

  • Conduct regular follow-up to assess service quality and client satisfaction.
  • Identify areas for improvement and implement corrective actions where necessary.
  • Participate in audits, evaluations, and continuous improvement initiatives.

Qualifications & Requirements

  • GCE 'O'/'N' Level, Diploma in Nursing, Healthcare Management, Social Work, Gerontology, or related field.
  • Prior experience in homecare, healthcare coordination, or eldercare services is preferred.
  • Strong understanding of care planning, case management, and healthcare systems.
  • Good interpersonal, communication, and problem-solving skills.
  • Ability to work independently and manage multiple priorities in a fast-paced environment.
  • Proficient in care management systems.
  • Fluency in English and Mandarin, able to speak simple conversational dialects such as Hokkien, Cantonese is an advantage, especially for client engagement.
