Key responsibilities:

  • Review clients' records and application history to understand their medical and care backgrounds.
  • Conduct in-depth interviews with clients across settings (e.g., hospitals, homes) to assess medical, psychological, social, and functional needs.
  • Perform assessments using tools such as interRAI, RAF, FAR, MBI, AMT, CFS, and interpret findings using clinical judgment.
  • Draw insights from assessments done by other professionals (e.g., MMSE, GDS) to identify care issues or red flags.
  • Work closely with multidisciplinary teams to develop or refine care plans that best support clients' needs.
  • Implement interventions and ensure follow-up care is appropriate and timely.
  • Promote and assist in the administration of MOH programmes, schemes, and community services, guiding clients through application processes.
  • Educate clients and caregivers on condition management, self-care, and community support resources.
  • Act as a key point of contact for clients and caregivers, offering guidance and referrals where needed.
  • Contribute to continuous service and process improvements as a subject matter expert.

Requirements:

  • Degree or professional qualification in Nursing, Social Work, Counselling, Allied Health, or related disciplines.
  • At least 3 years of experience in healthcare settings (acute and/or community).
  • Knowledge of geriatrics, care coordination, and community services is advantageous.
  • Strong communication and interpersonal skills, with a client-centred approach.
