What We're Looking For
Trillium Health Resources has a career opening for a Support Transition and Reentry (STAR) Clinician for the STAR specialty care team serving incarcerated adults with complex behavioral health conditions. The Clinician is responsible for providing clinical guidance to the team, leads regular clinical staffings, assesses level of care needs, develops transition plans, and links members to clinically appropriate services. The ideal candidate has a wide range of clinical expertise, experience working with adults with complex behavioral health conditions, and has considerable knowledge of criminal justice systems, reentry programs and community-based services.
On a typical day, you might:
- Complete assessments and coordinate care for assigned individuals.
- Collaborate with internal staff to facilitate complex transitions while ensuring integrated care.
- Monitor the care plan, service delivery, post-transition clinical needs, and health and safety.
- Perform clinical functions of discharge/transition planning and diversion.
- Provide education about all available services and natural and community supports, treatment options, diagnosis, etc.

Qualifications
Required:
- Master's degree.
- Minimum one (1) year of relevant experience working directly with individuals with Serious Mental Illness (SMI); or must have one (1) year prior relevant and direct experience providing diversion services under TCL.
- Fully licensed by the state of North Carolina as an LCSW, LCMHC, LPA, or LMFT.
- Must have a valid driver's license.
- Must reside within Trillium's catchment area of North Carolina, which includes the following counties: Anson, Bertie, Beaufort, Bladen, Brunswick, Camden, Chowan, Carteret, Columbus, Craven, Currituck, Dare, Duplin, Edgecombe, Gates, Greene, Guilford, Halifax, Hertford, Hoke, Hyde, Jones, Lee, Lenoir, Martin, Montgomery, Moore, Nash, New Hanover, Northampton, Onslow, Pamlico, Pasquotank, Pender, Perquimans, Pitt, Randolph, Richmond, Robeson, Sampson, Scotland, Tyrrell, Warren, Washington, Wayne, Wilson
- Must be able to travel within catchment as required.
Preferred:
- Degree in Human Services or Criminal Justice.
- Minimum one (1) year working in criminal justice or juvenile justice systems.
- Minimum two (2) years post-Master's experience in a clinical setting with the MH/SU population.
- Experience working with individuals with severe and persistent mental illness who are living in an institutionalized setting.
