The Job:

The Right-siting Officer will actively support the right-siting of clinically stable patients from respective specialist outpatient clinics (SOCs) to primary healthcare providers. He/she will assist the SOC clinicians in counselling and motivating discharged patients to continue their care in primary care setting. He/she has to be patient-centred with good communication skills.

Your job roles will include, but not be limited to the following:

·       Providing assistance to SOC clinicians in explaining the available options for follow-up at primary care.

·       Address queries, counsel and motivate patients regarding their follow-up care in primary care.

·       Contact GP clinic for booking of follow-up appointment.

·       Update patient records and maintain the upkeep of the database for the right-sited patients.

·       Follow up calls to right-sited patients to ensure that they are satisfied with the care provided at the DOT GP

·       Any other ad-hoc projects as required.

Requirements:

·       Minimum GCE O Level and above.

·       Nursing / patient service / healthcare background will be an added advantage.

·       Fluent in English and preferably Mandarin.

·       Ability to speak other languages, especially dialects, will be an added advantage.

·       IT savvy and proficient in using Microsoft Office.
