What We're Looking For

Trillium Health Resources has a career opening for a Regional Care Team (RCT) Support Specialist to join our Care Management team. Trillium's Regional Support Specialist role is integral to the Care Management department, providing key support to the Regional Management Care Team by providing confidential administrative and operational support. Additionally, the Regional Support Specialist assists the Clinical Care Management Director with tracking referrals to the Regional Care Teams, triaging calls, and assigning members to the Regional Care Managers. The Regional Support Specialist will also support member outreach and engagement activities as needed.

On a typical day, you might:

  • Assist with data entry, reconciling, tracking, and monitoring of internal data collection and State reports related Tailored Care Management.
  • Review and track referrals to support the RCT teams.
  • Assist with performing outreach, engagement, and follow-up to members/guardians, as needed.
  • Provide administrative, clerical, or custodial oversight or action to resolve day-to-day issues.

Qualifications

Required:

  • High School diploma/GED
  • Minimum of one (1) year clerical experience OR equivalent combination of education/experience. 
  • Must have a valid driver's license.
  • Must reside in North Carolina within Trillium's catchment area, which includes the following North Carolina counties: Bertie, Camden, Chowan, Currituck, Gates, Halifax, Hertford, Martin, Northampton, Pasquotank, Perquimans, Warren, Beaufort, Dare, Edgecombe, Greene, Hyde, Nash, Pamlico, Pitt, Tyrrell, Washington, Wilson, Carteret, Craven, Duplin, Jones, Lenoir, Onslow, Pender, Sampson, Wayne, Bladen, Brunswick, Columbus, Hoke, Lee, Moore, New Hanover, Robeson, Scotland, Anson, Guilford, Montgomery, Randolph, and Richmond. 
  • Must be able to travel within catchment as required.

Preferred:

  • Experience in assisting a senior leader/executive and four years of clerical and office management experience.
