As Senior/ Executive, he/ she will be part of the Group Allied Health team, working closely with NHG internal stakeholders, and community partners to support the effective implementation of the One-Rehab Framework in NHG.
The key responsibilities include supporting projects as part of organising place-based and person-centric rehabilitation care for patients; improving outcomes; and improving integration and collaboration between rehabilitation settings. In addition, he/ she will provide operational support at assigned operating unit(s)/ area(s), service or program in Rehabilitation Departments across NHG institutions in data monitoring and evaluation of One-Rehab program.
SPECIFIC (70-80%)
Support the improvement in delivery of rehab care and patient journeys across the continuum of care under One-Rehab program
- Assist in planning and implementation of changes required to improve rehab care and patient flow
- Assist to identify barriers to change and develop structures/ levers/enablers to scale best practices in collaboration with healthcare team
- Support review of policies and processes to achieve effective value-driven rehab outcomes throughout the continuum of care
- Foster, manage and mobilise change among internal and external stakeholders e.g. community partners across the continuum of rehab care
- Provide support in change management issues arising from adoption of the One-Rehab Framework
- Coordinate care for patients with complex needs to navigate the rehab journey to ensure continuity of care and to prevent patients from dropping out of rehab prematurely
Provide operational support of One-Rehab program
- Ensure smooth daily operations of the assigned operating units/areas, services or programs under One-Rehab
- Administer collection of key data components from patients/ clients under specific guidance and training
- Enter, extract, process, compile data, statistics and reports for regular reporting to stakeholders (e.g. MOH, AIC)
- Assist in drafting reports, papers and other documents as required
