**Roles & Responsibilities**

You will be part of the team in coordinating and managing the care of chronic kidney disease patients. You will evaluate, plan and implement care of patients with chronic kidney disease, collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services. You will also maintain the related database and provide continuous and relevant updates, and submission of reports to the Programme Manager and/or Renal Champion Doctor where required. The incumbent will also assist to review and develop patient education materials in consultation with physicians or other health-care providers where required, and to assist in reviewing and evaluating work processes in consultation with physicians or other health-care providers where required. You will also precept newly joined coordinators in delivery of patient care where needed, participate in quality improvement activities and take on any other ad-hoc tasks as assigned by your Reporting Officers.

**Requirements**:

- Bachelor Degree in Nursing or Bachelor Degree in Life Science, Medical Sciences and/or related medical fields.
- Team player with excellent interpersonal and communication skills.
- Able to converse in English, Mandarin, Malay or major local dialects (in order to liaise with the English, Mandarin, Malay & major local dialects speaking associates).
