**MAJOR DUTIES AND RESPONSIBILITIES**:

- **SPECIFIC (90%)**
- Case management for early and advanced chronic kidney disease (CKD) patients to delay their disease progression through education.
- To coordinate peritoneal dialysis (PD) catheter insertion/removal with nephrologist/urologist, anaesthetist nurse and PD nurse for both inpatient and outpatient.
- To coordinate with PD nurse and patient on PD training date post Tenckhoff catheter insertion.
- **To monitor and track of patient’s clinical status to ensure smooth transition during conversion from HD to PD and vice-versa.**
- To facilitate interim haemodialysis (HD) placement at chronic HD centre by working with patient and caregivers.
- To ensure timely submission of medical documents in Integrated Referral Management System (IRMS) as requested by VWO.
- To conduct Advance Care Planning (ACP) discussion for renal patients in the inpatient and outpatient setting.
- To coordinate Holistic Approach in Lowering & Tracking CKD (HALT-CKD) patient referrals and data entry.
- To submit monthly HD and PD patient statistics to National Registry Disease Office (NRDO).
- To collate statistics from renal centre, renal coordinators, operations, MSWs for monthly submission of department Balance Score Card (BSC).
- To obtain the necessary clinical and administrative information for audit and quality improvement for procedures done by nephrologists.
- To collate data for yearly department report and presentation.
- To assist nephrologist in clinical research and ensure adherence for protocol procedure.
- To explain protocol to patients.
- To ensure that informed consent is taken as per SGGCP requirements.
- To trace and returning of case notes, blood results & all other results required in the protocol.
- To contact and schedule appointments for patients within the time frame required in the protocol.
- To centrifuge blood specimens and send specimens to both local and overseas central laboratory.
- To file and document clinical trial files and electronic case report forms.
- To maintain source documentation for each patient according to protocol requirements.
- To inform PIs and collaborators on any dose adjustments based on protocol change.
- To collect returned study products by patients and keep accountability logs.
- To identify and document Adverse Events (AE) and obtain the PI’s signature.
- To identify, document and submit protocol deviations to Domain Specific Review Board (DSRB) according to hospital and sponsor procedures.
- To ensure systematic filing and maintenance of work procedures and research results.
- To ensure maintenance of research study database.
- Assist investigators in simple administrative reports.
- **GENERAL (10%)**
- Administrative, data collection and document accurate minutes of meeting.
- Assist department in the bi-yearly department outreach events.
- Perform any other ancillary duties as assigned by the reporting officer.

**EDUCATION, TRAINING AND EXPERIENCE**
- Diploma in Nursing or healthcare related
- 2 years working experience, preferably with renal related experience.
- Good PC knowledge is an advantage.
- Possess positive attitude and consistent good performance.
- Possess good interpersonal and communication skills.
- Possess positive attitude, initiative and keenness to learn.
- Able to work independently.
- Be proactive and able to implement the change.
