**Job Description**:
The Challenges
1. Responsible for operational support and continuous improvement of assigned track(s), operational unit(s)/ area(s)/ service(s) and/or project(s) in Community Health
2. To support initiatives, projects or services in Community Health, ensuring that all relevant KPIs are met
3. Ensure smooth daily operations of the assigned services or programmes by working closely with stakeholders (internal & external)
4. Anchor engagements and build relationships with care partners in the community for collaborations
5. Ensure that relationships with care partners (internal & external) are maintained across the continuum of care
6. Track relevant datasets and assist in the analysis of reports
7. Assist in submission of data and reports to stakeholders e.g. MOH, AIC as part of regular reporting
8. Assist in planning and implementation of changes required to improve service levels and raise quality standards
9. Ensure that volume and financial targets for the assigned services or programmes are achieved
The Requirements
1. Degree in any discipline
2. At least 5 to 7 years of work experience, preferably in a healthcare setting
3. Ability to exercise lateral thinking and innovation
4. Possess good interpersonal and communication skills
6. Relevant or related work experience is an added advantage
